In order to give you exceptional care, it is important to complete your demographics, new patient questionnaires/forms & enter your current medications, supplements/vitamins, allergies and preferred pharmacy in the corresponding tabs, prior to your appointment.
You may also upload any pertinent recent lab, radiology or physician reports into your chart in the labs & document tab, if you feel it is relavant for me to review to help assess your condition.
If you are being seen for a follow up appointment, please complete the 2 questionnaires under the follow up header and be sure to update any change in your meds, supplements, allergies, etc. prior to your appointment.
If you are being seen for a specific condition, please complete the correspnding forms that pertain to that condition under the additional form section.
Lastly, you may also enter a vital sign that you may be monitoring from home, like pulse, blood pressure, weight, height and waist circumference prior to your appointment.
